Sinus surgery, from the foundations to advanced case management

Develop your skills from standard lateral and crestal sinus elevation to demanding anatomy, alternatives to sinus augmentation and complication management.

Learn lateral and crestal sinus elevation, train on demanding anatomical models and watch Pedro Moura perform three planned surgeries on real patients. Then take the next step: choose supervised clinical training and operate with us at Foramen Headquarters in Porto, Portugal.

From your first sinus cases to difficult anatomy and alternatives for the posterior maxilla, develop the technique and judgement to approach treatment with greater control. Through detailed clinical cases, explore membrane perforation, bleeding, implants displaced into the maxillary sinus and oroantral communications.

Sheet 03 · The details

Learn to handlethe details that make the surgery work

Knowing the sequence of a sinus lift is the beginning. The clinical work lies in adapting that sequence to the patient: where to make the incision, how much access you need, how to approach a septum, which instrument gives you the movement you need and what to do when the membrane behaves differently from what you expected.

This course opens up that work. Pedro explains the rationale behind his current protocols and the refinements developed through his clinical practice. You examine ordinary cases closely, rehearse more demanding anatomy and follow the decisions in live surgery.

The aim is to develop a more deliberate, controlled approach to sinus augmentation, with a clearer understanding of each operative step and the alternatives available when the original plan needs to change.

Sheet 04 · Live day

Train with the teamwho will be beside you

The live day gives you access to the working method around the surgery as well as the surgery itself.

Review how we prepare a case, organise the surgical sequence, select instruments and biomaterials, coordinate with the team and plan postoperative care.

At each demonstration, follow the link between the scan, the operative field and the decisions being made. Ask why one instrument is used rather than another, why the access changes between patients and what information would make us continue, modify the approach or stop.

Train at Foramen Headquarters, where surgical care and education share the same clinical setting. Work with the team, instruments and case preparation around the live procedures, then explore Porto between course days.

Sheet 05 · Clinical reasoning

The reasoningbehind our protocols

Fig. 05 · Field Notes plate: planning the antrostomy

05.1

Assess the patient with a purpose

Use the preoperative questionnaire to identify the history and symptoms that change planning. Connect that information to your radiographic findings and define the next action: proceed, modify treatment, treat first or investigate and refer.

05.2

Read the anatomy as a surgical plan

Examine the available bone, lateral wall, vessels, floor, septa and membrane appearance. Understand how these findings change the access, instruments and elevation. Discuss the limits of the scan and the role of drainage assessment.

05.3

Choose the appropriate approach

Compare lateral and crestal elevation and decide when implants can be placed simultaneously or treatment should be staged. Consider relevant alternatives for the posterior maxilla and explain why a sinus procedure belongs in the patient’s plan.

05.4

Prepare for a change in the operation

Work through membrane tears, difficult elevation, bleeding and problems with graft containment. Follow a membrane-repair demonstration and discuss when the better decision is to change the approach or stop and return.

Sheet 06 · Posterior maxilla alternatives

Beyondsinus augmentation

Learn when to elevate the sinus, and when to consider another route.

Explore trans-sinus, pterygoid, palatal and zygomatic options through a focused comparison of real clinical cases. Understand what makes an option relevant, what limits it and how the restorative plan changes the decision.

Then bring one of those options into practice: rehearse a trans-sinus implant placement on a model using a bone level guide. Connect the scan and planned implant position to the guide and the operative steps, with instructor feedback.

The theoretical module covers all four alternatives. The additional hands-on exercise focuses on trans-sinus placement.

Create a surgical guide for sinus surgery

Turn the selected case plan into a surgical guide. Work through its creation and check its fit and relationship to the planned access on the model.

Ex. 02

Lateral window with bony repositioning

Prepare and remove the bony window, elevate the membrane and rehearse graft placement and repositioning of the bony lid.

Ex. 03

Standard lateral window

Prepare the lateral access, elevate the membrane and work through graft containment and management of the window.

Ex. 04

Crestal sinus lift

Rehearse osteotomy preparation, floor crossing, elevation and implant placement, including osseodensification.

Ex. 05

Sinus surgery with septa

Adapt the access and instrument movements to the anatomy of a model case with septa.

Ex. 06

Trans-sinus implant placement

Work through a planned trans-sinus case on a model, connecting the anatomy, implant trajectory and operative execution.

Membrane repair, selected palatal entry techniques and the role of endoscopic inspection are addressed through the related teaching and demonstrations.

Fig. 08 · Pedro Moura, lateral sinus lift (1:28)

Sheet 08 · Day 2 · Live surgery

Follow threedifferent decisions in live surgery

The second day is planned around two sinus augmentation procedures and a third case demonstrating either an alternative to sinus augmentation or a more complex sinus procedure. The sinus cases may include lateral, crestal or demanding anatomical situations, selected to show different treatment decisions.

The actual procedures follow the patients’ needs; no specific alternative operation is guaranteed for every edition. Complication teaching is supported by demonstrations and recorded clinical cases, so membrane repair and re-entry decisions remain part of the course whatever happens during the demonstrations.

BEFORE

You review the patient’s history, scan and treatment plan, together with the planned access and instrument sequence.

DURING

Pedro explains the fine details of exposure, membrane handling, graft placement and closure, including the decisions that are easy to miss when watching an edited video.

AFTER

The group compares the plan with the operation and reviews implant timing and postoperative management.

Sheet 09 · Programme

Your two daysin Porto

Day 1

Assessment, planning and hands-on surgery

Morning sessions connect sinus anatomy, clinical history and CBCT interpretation to approach selection, including preparation for the trans-sinus exercise. The afternoon includes six hands-on exercises: sinus guide creation, lateral window with bony repositioning, standard lateral window, crestal lift, a septa case and a trans-sinus case. Related teaching addresses membrane repair and selected palatal entry techniques. The day ends with the planning conference for the live patients.

Day 2

Posterior maxilla alternatives and live surgery

Start with a focused, case-based overview of trans-sinus, pterygoid, palatal and zygomatic options. Then follow the three planned procedures from case briefing to final debrief: two sinus cases and a third alternative or complex sinus case. Discussion includes grafting, implant placement, medication rationale, postoperative care and the response to complications.

Day 3 · 25 Mar 2027 · Optional

You operate on a patient

Review and plan your allocated case with Pedro Moura, then perform your agreed operative role under direct supervision, followed by a clinical debrief.

Only 6 places. Add it at checkout when you book the course.

Your printedOperative Manual

Bring the Field Notes approach into your sinus surgery. Connect illustrated concepts with operative steps, practise those steps on models and follow them in real patient treatment.

Every participant receives a printed, course-specific Field Notes Operative Manual at registration in Porto. The manual is yours to annotate during training and keep afterwards; course handouts are supplied in print.

The Sinus manual brings together patient assessment and radiographic review sheets, operative sequences, instrument and material planning, membrane-repair reference sheets, posterior maxilla treatment comparisons, sinus guide-creation worksheets, trans-sinus planning and guide-check sheets, live-case notes and postoperative review prompts.

Use it to record the details that change your execution: the incision, the access, the instrument movement and the decision to adapt. Leave with the material you worked through, your own annotations and the cases discussed during training.
